What the published literature examined
Research has examined tesamorelin in randomised, double-blind, placebo-controlled trials in adults with HIV and abdominal fat accumulation, with visceral adipose tissue measured by computed tomography over 26 weeks and over 12 months. The same trials measured triglycerides, cholesterol ratios, IGF-1, fasting glucose and insulin. A separate randomised multicentre trial examined hepatic fat fraction by magnetic resonance spectroscopy in adults with HIV and non-alcoholic fatty liver disease. Pharmacology work has characterised the compound as a 44-amino-acid analogue of growth hormone-releasing factor carrying an N-terminal hexenoyl modification.
What is not established
Long-term cardiovascular safety is not established, and the approved label states the product is not indicated for weight loss management.
What distinguishes Tesamorelin
Tesamorelin is the only growth hormone-releasing hormone analogue holding a live US marketing authorisation. Every other GHRH-family peptide in this category has none. Because it is a 44-amino-acid peptide it crosses the FDA protein threshold, so in March 2020 its application was deemed a Biologics License Application: it is regulated as a biologic, which almost nothing else in this catalogue is.
